All Service Requests are billed by the hour, with a one hour minimum charge, per technician. Any additional hours will be billed at a pro-rated rate. Service Rates include travel time. Time of Service begins when the technician leaves the office or previous appointment.

This form serves as authorization to charge your card for the 1st hour of service and any equipment being installed. Should any additional parts or labor be necessary, you will have the option of paying the balance by check or credit card. However, if your account becomes past due, WE RESERVE THE RIGHT TO CHARGE YOUR CREDIT CARD FOR ANY BALANCES OWED TO US, plus a 3% credit card processing fee.
